In the Link for the Lazy, 90% of the time I include the "patch notes" version. I always keep a written record of the changes to the map, its why the templates need to be exact and organized.

It's not often I post notes, but when there's a big event like a cloudbreak, I make sure I note it at the end of the template so I know my own thoughts when I refer back to it. Probably should've mentioned that it broke the Dominion, I'll do a better job of that next round.
