Any impact with the front face releases powerful repulsor energies, as the deadly power hammers are want to do in mining operations. This force is enough to toss even a fully armored mandalorian or heavy battle droid through the air with full contact, and lighter armored foes will find bones shattered and organs bruised, at the very least. A full on impact blow to the head of someone without a helmet would be quite nasty.
The backspike is another matter entirely, and a blow for it, just like from the blastsword it was based on, releases a torrent of energy. This energy, however, is highly focused and deadly to electronics less so than lethal to organics, as it is pure ion energy. Though, the likely effect of a large spike of durasteel being driven through your armor/body is damage enough to organics in any case.
Additionally, though it will cause little damage to typical hearing, each strike of the hammer on either side is accompanied by rumbles and roars and pops of ionization and repulsor energy release, making the swinging of this hammer sound for all the worlds like a rumble of thunder.
