Critical miscalculation, but not something that was unlikely. Niamh was not unfamiliar with Force illusions, nor was she unfamiliar with the trickery of Sithlings. That said, she'd still miscalculated the fact that this was an illusion and not the real Sith Lord. When her blade sliced through nothingness, her eyes went wide and she spun around to face her attacker head on, the lightning crackling from his fingertips towards her. This was not going to be pretty.
Of course neither was the giant worm thing, but that was a whole different matter.
Still, Janus had made a mistake as well. He'd assumed that he was only going to have to fight Niamh, forgetting about the other woman that had been with her and was slaying the crap out of his little peons when they got too close. So when he'd appeared behind Niamh, and not in front of her, and Teria had noticed this, she'd moved. While the lightning flickered out against Niamh, Teria flung herself at Janus, twirling her blades this way and that in an effort to at least throw Janus off enough to make the lightning stop tormenting and burning her charge. All she needed was to give Niamh a temporary respite.
